To help your first visit run smoothly, our team at Case PT asks that you print and fill out our patient forms, which can be conveniently found on our website. Be sure to bring them with you when you arrive for your physical therapy appointment.

Forgot to fill out the forms? No worries! You can also fill out the packet in our office — though we ask that you arrive 10-15 minutes early to do so. This makes sure your appointment can start on time.
